# Form field types

The right field type makes a form faster to complete and easier to review.

Use the simplest field that matches the answer you need.

### Available field types

* **Header** — adds a section title inside the form
* **Text field** — captures short or long written answers
* **Time** — captures a time only
* **Date and time** — captures both values together
* **Date** — captures a date only
* **Numeric** — accepts numbers only
* **Checkboxes** — lets employees tick multiple options
* **Radio buttons** — lets employees choose one option
* **Drop-down list** — shows one selectable list of options
* **Signature** — collects an employee or customer signature
* **Photo** — lets employees add photo proof
* **Table** — captures repeated values in rows or columns
* **Text paragraph** — captures a larger text block

### How to choose the right field

Use **radio buttons** when only one answer is allowed.

Use **checkboxes** when several answers can be selected.

Use **photo** or **signature** only when proof or approval matters.

### A note about text paragraph

A **Text paragraph** can be used for longer notes.

It can also be hidden in PDF output when you only need internal notes.

## Querying This Documentation
If you need additional information that is not directly available in this page, you can query the documentation dynamically by asking a question.

Perform an HTTP GET request on the current page URL with the `ask` query parameter, and the optional `goal` query parameter:

```
GET https://knowledge.out-smart.com/marketplace-packages-and-power-ups/power-ups/forms/form-field-types.md?ask=<question>&goal=<endgoal>
```

`ask` is the immediate question: it should be specific, self-contained, and written in natural language.
`goal` is optional and describes the broader end goal you are ultimately trying to accomplish on behalf of the user. GitBook uses it to tailor the answer towards what is most useful for that goal.

The response will contain a direct answer to the question and relevant excerpts and sources from the documentation.

Use this mechanism when the answer is not explicitly present in the current page, you need clarification or additional context, or you want to retrieve related documentation sections.
